NP Outreach - Making a difference

Compass is a non-profit community based organization that provides mental health, developmental and community services to children, youth, and families in the district of Sudbury and Manitoulin.

COMPASS is funded by the Ministry of Health through the Centre for addiction and Mental Health to operate the Hub, which aims to deliver walk-in services in addictions, mental health, primary care as well as other social, vocational and community services for youth aged 12-25 years out of the Greater Sudbury YMCA.

SDNPC will provide primary health care services to the clients of the Hub. A Nurse Practitioner (NP) will be the Most Responsible Primary Care provider and will provide comprehensive, accessible, person-centred and co-ordinated primary care services through an inter-professional team approach.
